With over 1,000 beds available across 30 locations, we are focused on
delivering high-quality, comprehensive care and support to
homeless or displaced individuals in need.
As a DHCS CalAIM Community Supports provider, we serve individuals who are
transitioning out of medical facilities or are at risk of hospitalization.
Our goal is to offer a safe, supportive, and structured environment with short-term
housing and individualized care-including clinical and medication oversight.
Services We Offer
- 24/7 security and supervision
- 24/7 on-site management
- Three hot and nutritious meals per day
- Cable TV and high-speed internet
- Communal areas for relaxing and socializing
- Complimentary laundry services
- Coordination with clinicians and medication adherence
- Coordination with case management and housing transition service providers
- Assistance with applying for housing assistance programs, DMV, and new benefit programs
Eligibility for Services:
Housing Status
(only needs one factor below to be eligible):
- Homeless or at-risk of homelessness
- Facing housing insecurity or instability
- Living in unsafe or unsuitable conditions
- Lives alone without support
- Current housing threatens health and safety Recent Discharge
- Recent hospitalization discharge, OR
- Recent inpatient skilled nursing or psychiatric stay Medical Condition (only one of the following is required):
- Needs to heal from an injury or illness
- Needs to stay medically stable to avoid hospital readmissions
- At-risk for hospitalization due to an unsafe living environment
- Needs short-term help with activities of daily living (ADLs)
- Requires ongoing monitoring of health or behavioral conditions (e.g., vital signs, wound care, medications)
Medical Condition
(only needs one factor below to be eligible):
- Post-hospitalization or discharge from a medical setting
- Needs to heal from an injury or illness
- At-risk of hospitalization due to an unsafe living environment
- Needs to achieve or maintain medical stability and prevent a hospital admissionor readmission
- Limited or short-term assistance with ADLs /IADLs
- Needs ongoing monitoring of medical or behavioral health condition(s) (i.e. monitoring vital signs, assessments, wound care, medication monitoring)

Life at Soul Housing
Each one of our facilities features beds and linens, three hot and nutritious meals per day, cable TV, high-speed internet, communal areas for relaxing and socializing, ADA accessible bathrooms and showers, and complimentary laundry facilities. Our residents are free to come and go as they please.
